چکیده

Subchondral bone marrow lesions (BMLs) are related to structural and symptomatic osteoarthritis progression. However, it is unclear how sequence selection influences a quantitative BML measurement and its construct validity. We compared quantitative assessment of BMLs on intermediate-weighted fat suppressed (IW FS) turbo spin echo and 3-dimensional dual echo steady state (3D DESS) sequences. We used a customized software to measure 30 knees' (24- and 48-month MR images) BMLs on both sequences. The results showed that the IW FS sequences have much larger BML volumes (median: IW FS = 1840 mm(3); DESS = 191 mm(3)) and BML volume change (between 24 and 48 months) than DESS sequence and demonstrate more BML volume change. The 24-month BML volume on IW FS is correlated with BML volume on DESS (r s = 0.83). BML volume change on IW FS is not significantly correlated with change on DESS. The 24-month WOMAC pain is correlated with the 24-month BMLs on IW FS (r s = 0.39) but not DESS. The change in WOMAC pain is correlated with BML volume change on IW FS (r s = 0.37) but not DESS. Overall, BML quantification on IW FS offers better validity and statistical power than BML quantification on a 3D DESS sequence.

Clinical and Pathophysiologic Significance of MRI Identified Bone Marrow Lesions Associated with Knee Osteoarthritis

Knee osteoarthritis (OA) affects the joint beyond just the articular cartilage. Specifically, magnetic resonance imagingidentifiedbone marrow lesions (BML) in the subchondral bone have both clinical and pathophysiological significance.Compared to joint space narrowing on traditional radiographs, the presence of BMLs has been better correlated withseverity of clinical symptoms ...

The Healing Effect of Bone Marrow-Derived Stem Cells in Knee Osteoarthritis: A Case Report

Osteoarthritis (OA) is a prevalent chronic disease impacting on quality of life and has societal and economical burden increasing with age. Yet, no confirmed pharmacological, biological or surgical therapy could prevent the progressive destruction of OA joint. Mesenchymal stem cells (MSCs) with immunosuppressive activities emerged a potential therapy. We describe a magnetic resonance images (MR...

ارتباط میان یافته‌های کمّی شده MRI با یافته‌های بالینی و رادیوگرافی ساده در بیماران مبتلا به استئوآرتریت زانو

Background: Unlike quantified MRI, no correlation has been stated between radiologic findings and the clinical signs in patients with knee osteoarthritis. This study evaluates the relationship between quantified clinical signs including pain, restriction of movement, stiffness and structural changes with MRI and plain radiography findings. Methods: Eighty patients with knee osteoarthr...

Bone marrow edema-like lesions and cartilage degeneration in osteoarthritis using 3T MR T1rho quantification: longitudinal assessment

INTRODUCTION Bone marrow edema-like lesions (BMEL) are defined as areas of high signal intensity in T2-weighted, fat-saturated magnetic resonance (MR) images or in short inversion time inversion-recovery images. These lesions are present in knee osteoarthritis (OA) and acute knee injuries. While MR findings of BMEL are common, our knowledge concerning their natural history and significance is l...

Semiquantitative assessment of subchondral bone marrow edema-like lesions and subchondral cysts of the knee at 3T MRI: A comparison between intermediate-weighted fat-suppressed spin echo and Dual Echo Steady State sequences

BACKGROUND Choice of appropriate MR pulse sequence is important for any research studies using imaging-derived data. The aim of this study was to compare semiquantitative assessment of subchondral bone marrow edema-like lesions and subchondral cysts using intermediate-weighted (IW) fat-suppressed (fs) spin echo and Dual Echo Steady State (DESS) sequences on 3 T MRI. METHODS Included were 201 ...
